When should I arrive for my first appointment?

Your first appointment at Flow Physical Therapy in Chicago will be a comprehensive evaluation with a licensed physical therapist. Please arrive 15 minutes before your scheduled appointment time. There may be additional forms to complete in our office, such as consent forms and a functional survey. 

What should I bring to my first appointment?

Please bring the following with you to your first appointment:

  • Issued Photo ID (valid driver's license, state ID, passport)

  • Insurance Cards

  • Please read our Notice of Privacy Practice and complete your Patient Intake Forms for your first appointment.

  • Please wear comfortable and suitable clothing for exercise.

  • Your schedule to make future appointments.

It is your responsibility to know your coverage and we highly recommend that you check your benefits as well.
